  • Introduction: Stroke or cerebrovascular accident is the third leading cause of mortality in the world. It has been reported that vitamin D deficiency is a risk factor for cerebrovascular accident, and has a neuroskeletal and neuroprotective effect that may decrease functional and cognitive impairment in stroke patients.
  • Methods: In this double blind randomized clinical trial, 64 patients referred to the Neurology clinic in valiassr Hospital of Arak university of Medical sciences, Iran , with a diagnosis of acute ischemic stroke and vitamin D serum level lower than 50 nmol/l, were randomly divided into two groups of 32 people. In all patients, demographic and clinical information and a questionnaire for assessing the severity of neurological complications (CNSS) were completed. Then the first group received routine stroke cares for 3 months (ASA80mg daily + plavix75mg daily), also received 50,000 units / week of vitamin D, and patients in the second group received only routine stroke treatment and placebo. None of the patients and physician were aware of the allocation of the groups. At the end of the third month, the CNSS questionnaire and serum vitamin D3 level were re-measured.
  • Results: The mean age of patients was 66±14 years, and the most common risk factor was hypertension. The ischemic stroke was the most common type and most of the lesions were found in the anterior circulation territory. Finally, it was found that the severity of the neurological complication was not significantly different between the two groups after weekly vitamin D3 administration for 3 months
  • Conclusion: Weekly administration of 50,000 IU vitamin D3 for 3 months, as a complementary therapy in patients with acute ischemic stroke, has not significant effect on severity of the neurological complication.
