Abstract


Introduction

Ventilator associated pneumonia(vap) is one of the most common hospital acquired infections that occurs approximately 48 to 72 hours after the patient receives mechanical ventilation. early diagnosis of vap can reduce the emergence of resistant organisms .we assess of the common pathogens in vap by multiplex pcr

Methods

We carried out on 21 bal samples of vap patients hospitalized for more than 48 hours in icu of loghman hospital .we used multiplex pcr for detection of specific genes lyta for streptococcus pneumoniae , its region for the acinetobacter baumannii and hsp60 for enterobacter cloacae.

Results

Among the 21 samples, the following were detected acinetobacter baumannii 9 cases (42.8%), enterobacter cloacae 1 case (4.7%) . we found mixed infection in vap samples: acinetobacter baumannii and enterobacter cloacae 3 cases(14.3%), acinetobacter baumannii and streptococcus pneumoniae 1 case(4.7%), acinetobacter baumannii , enterobacter cloacae and streptococcus pneumoniae 1 case(4.7%) , enterobacter cloacae and streptococcus pneumoniae 1 case(4.7%) and negative 5 cases(23.8%).

Conclusion

Multiplex pcr is a rapid test for diagnosis of vap. early detection of micro organisms has the potential to facilitate treatment decisions. we found that acinetobacter baumannii was the most prevalence in our samples and interestingly 28.5% of our samples were mixed infection.
