Evaluation of h.pylori drug resistance to current cure in north-west of iran

Abstract


Introduction

Almost half of the world population is infected with helicobacter pylori, a major risk factor for chronic gastritis, peptic ulcer and gastric cancer. the most effective way to improve the eradication of h. pylori peptic ulcers is recommended. one of the main reasons for the failure of helicobacter pylori is resistance to antibiotics. the purpose of this study was to determine the antibiotic susceptibility of helicobacter pylori strains prevalent in the northwest of iran to 8 antibiotics.

Methods

From 450 patients who referred to imam reza hospital in tabriz, gastritis complained, endoscopy was performed then biopsy specimens were prepared and further confirmed by rapid urea test, followed by sensitivity to the eight antibiotics commonly used in the treatment of helicobacter pylori infection, including clarithromycin, nitrofurantoin, amoxicillin, rifampin, ciprofloxacin, metronidazole, erythromycin and tetracycline by disk diffusion agar, and sensitivity to three antibiotics tetracycline, metronidazole and clarithromycin were investigated using an epsilometric test method.

Results

H.pylori was isolated from 139 samples (90 %) of the total of 450 biopsies.resistance to clarithromycin, nitrofurantoin, ciprofloxacin, metronidazole, tetracycline, rifampin, erythromycin and amoxicillin was 18%, 8%, 39%, 79% 13%, 29%, 27%, 24% respectively which was determined by the disk diffusion method.

Conclusion

Comparison of the data with previous results showed that the prevalence of h.pylori resistance to ciprofloxacin and metronidazole is rising sharply in the present study, the lowest resistance to tetracycline and nitrofurantoinwasobserved. the study showed that in order to determine the appropriate drug regimen against helicobacter pylori, we are always in need of antimicrobial susceptibility test.
