Abstract

Introduction

Gestational diabetes mellitus (gdm) is one of the most endocrine pregnancy outcomes. high prevalence of gdm was reported in infertility and assisted reproduction technology (art).both gdm and art have several adverse effect on both mother and her offspring. the association between pre-gravid bmi with gdm was reported. the aim of this study was to determine the association between pre-gravid bmi with fasting glucose and lipid profile in gdm following art.

Methods

a nested case-control study was performed among 260 singleton pregnant women conceived through art and natural conception. gdm was detected by a one-step ogtt at 2nd trimester of gestation. all participants were included after written informed consents and classified to 4 groups according to mode of pregnancy (art and non-art) and ogtt (gdm+ and gdm-) , consisted of (art, gdm-art, gdm and control groups). pre-gravid weight, height and first trimester fbs was obtained from medical records. body mass index (bmi) was calculated. a fasting blood sample was taken at 2nd trimester for biochemical evaluation (fbs, lipid profile). the correlation between bmi and biochemical test was determined. p < 0.05 was considered significant.

Results

Of 260 women, 120 art subjects (54 gdm-art and 66 art), and 140 natural conception (60 gdm and 80 control) were enrolled. the mean pre-gravid bmi in art, gdm-art, gdm and control were 24.1±3.5, 27.4±3.4, 25.8±5, 23.9±4 kg/m2, respectively (p<0.001). fbs (1st and 2nd trimester) was significantly higher in gdm-art and gdm than art and control group. bmi was significantly correlated with fbs 1st trimester (r= 0.23, p< 0. 001), fbs 2nd trimester (r= 0.29, p< 0.001), hdl(r= -0.16, p< 0.008). of 260 women, 120 art subjects (54 gdm-art and 66 art), and 140 natural conception (60 gdm and 80 control) were enrolled. the mean pre-gravid bmi in art, gdm-art, gdm and control were 24.1±3.5, 27.4±3.4, 25.8±5, 23.9±4 kg/m2, respectively (p<0.001). fbs (1st and 2nd trimester) was significantly higher in gdm-art and gdm than art and control group. bmi was significantly correlated with fbs 1st trimester (r= 0.23, p< 0. 001), fbs 2nd trimester (r= 0.29, p< 0.001), hdl(r= -0.16, p< 0.008). of 260 women, 120 art subjects (54 gdm-art and 66 art), and 140 natural conception (60 gdm and 80 control) were enrolled. the mean pre-gravid bmi in art, gdm-art, gdm and control were 24.1±3.5, 27.4±3.4, 25.8±5, 23.9±4 kg/m2, respectively (p<0.001). fbs (1st and 2nd trimester) was significantly higher in gdm-art and gdm than art and control group. bmi was significantly correlated with fbs1st trimester (r= 0.23, p< 0. 001), fbs 2nd trimester (r= 0.29, p< 0.001), hdl(r= -0.16, p< 0.008).

Conclusion

Higher pre-gravid bmi was correlated with increasing in fbs and low hdl in art and non-art subjects. pregnancy weight reduction is recommended for prevention of gdm especially before art treatment.
